چکیده
Introduction Computed Tomography Pulmonary Angiography (CTPA) scans have become readily accessible in healthcare to diagnose Embolism (PE). In real-world settings, there is variability practice patterns with a growing culture of more pragmatic approach related D-dimer testing and subsequent utilisation CTPAs. However, the issue low specificity remains, risking increased unnecessary scanning lower diagnostic yield. We aimed correlate outcome Methods This was single-centre retrospective observational study. collected data on all D-Dimer tests ordered by physicians rule out PE at high-volume acute cardio-respiratory unit. Correlation analysis conducted using Pearson coefficients or Spearman’s correlation, examine correlation CTPA Receiver operator curve (ROC) Area under (AUC) conducted. Results A total 1297 patients had serum levels done PE. The median age 58 years (IQR= 44-72). Only 0.9% (n=11) samples demonstrated D-dimers>20 while rest d-dimers 0.43 (IQR=0.28-0.98) ng/dL. 358 CTPAs were which 55 positive for (15%). Out D-dimers performed, 573 (44.2%) >0.5. Of these patients, 312 underwent (54.5%), 54 (17%) positive. 724 ≤0.5; 46 (6.4%) CTPA, one (2%) thromboembolic disease. There significant full cohort between (r=0.53, CI: 0.49-0.57). no statistically being utilised diagnosis (r=0.03). ROC AUC 0.79 P <0.001. d-dimer cut-off >0.14 yielded 100% sensitivity 72% undergoing our population. Conclusion study suggests over lack impact high likelihood false-positive results clinical context should be taken into consideration prior screening order avoid potential harm resource exhaustion that could result from well as incidental findings relevance controversial. Computed Tomography Angiography in Patients Evaluated for Acute Pulmonary Embolism with Low Serum D-dimer Levels: A Prospective Study.
CONTEXT Pulmonary computed tomography angiography (CTA) and the Wells criteria both have interobserver variability in the assessment of pulmonary embolism (PE). Quantitative D-dimer assay findings have been shown to have a high negative predictive value in patients with low pretest probability of PE. OBJECTIVE Evaluate roles for clinical probability and CTA in Emergency Department (ED) patien...
متن کاملImproving appropriate use of pulmonary computed tomography angiography by increasing the serum D-dimer threshold and assessing clinical probability.
OBJECTIVE To determine whether the implementation of an increased D-dimer threshold value and clinical probability assessment increases the prevalence of pulmonary embolism (PE) in patients undergoing pulmonary computed tomography angiography (PCTA) in an Emergency Department setting. METHODS A retrospective review of all patients undergoing PCTA during 2 separate 12-month intervals, 1 before...

OBJECTIVES A prospective outcome study designed to evaluate a simple strategy for the management of outpatients with suspected pulmonary embolism (PE), based on clinical probability, D-dimer, and multi-slice computed tomography (MSCT). METHODS A cohort of 432 consecutive patients admitted to the emergency department with suspected PE was managed by sequential non-invasive testing. Patients in...
متن کاملThe relationship between laboratory and chest computed tomography scan findings and severity of COVID-19 cases: a single center study
Background: Para-clinical abnormalities are considered as predictors of COVID-19 severity. We aimed at evaluate the relationship between laboratory and chest computed tomography (CT) scan findings and severity of COVID-19 cases. Materials and methods: We performed a retrospective study on confirmed COVID-19 patients in Amir-Al-Momenin hospital, Tehran, Iran, from February 20, 2020 to April 19,...
متن کاملCorrelation Between Tissue Densities in Computed Tomography and Three Different Cone-Beam Computed Tomography Units (In Vitro)
Background and Aim: Bone density is of great assistance in the selection of the proper implant site. The present study aimed to assess the correlation between tissue densities in computed tomography (CT) and three different cone-beam computed tomography (CBCT) units. Materials and Methods: In this descriptive study, a radiographic phantom consisting of a transparent polymethyl methacrylate (PM...
